Manage login identities as the master user

RightMV keeps login identities in the MVSYS USERS file. User and privilege administration is reserved for the configured master identity; entering MVSYS or setting a session option does not grant that authority. A new installation creates md with the initial password md, which must be changed immediately. Bootstrap settings create missing users and never reset an existing user or password.
